Healthcare Documentation

Any protection package you choose for you and your family must include healthcare documentation.  This may take the form of a Health Care Proxy, or Medical Power of Attorney (a document that allows you to designate someone to make medical decisions for you if you are unable), a Living Will (a document that allows you to choose which medical treatments you do or do not want if you are in a “end of life” situation) and a Designation of Guardian (a document that allows you to say who you would like to serve are your own guardian, both for property management and for personal care, if a court were ever to declare you incapable fo caring for yourself).

You must also provide for your child or children by creating medical authorizations for a non biological parent who has not yet adopted the child through a Second Parent Adoption.
